for the limited use herein specified and upon the terms and
conditions herein set forth, prints or transparencies of the

In consideration of our supplying you with the above, you
agree:
- To pay total fees of $_________, such payment to be made
in advance, with return of signed original of this
agreement. Your purchase order may be accepted by us in
lieu of advanced payment;
- To use the prints, digital image, or transparencies only
once, and then only for the purpose stated on this
form;
- To credit each reproduction with: Courtesy Fort
Collins Museum;
- Not to use or authorize others to use, alter, modify,
digitally enhance, or reproduce these items for ANY
purpose without written permission from us;
- To provide to the Museum two free copies of any produced
or published work containing photographs from our
collections.
- To assume all responsibility for any questions of
copyright that may arise.
Please sign and return the original copy of this agreement and
the Use Fee Form. Upon receipt of such signed copies and advance
payment, we will process your order.
